Separate sndfile.hh implementation into C++ source
...
This is required to be able to include
"audiographer/sndfile/sndfile_writer.h" in more than
one source file. which would otherwise lead to
duplicate symbol: AudioGrapher::SndfileHandle::..

...
NB. if someday we want to switch back to upstream
SndfileHandle, we need to subclass wrap it.
(not sure what else is different, anyway)

Fix export, which has been broken since the boost::signals2 changes. Also update Audiographer, bacause of its incomplete sndfile handling. Audiographer is equal to revision 74
