Clean up MonitorProcessorControls 
As opposed to regular AutomationControls these PBD:::Controllables
are not SessionObjects and don't emit a signal when the session
goes away.

e31f5d9998 Clean up State API: 
* Processor implement get_state(), classes derived from Processor
  implement protected ::state() -- as documented in processor.h
* likewise for Route, Track: make ::state() a protected interface
* removal of "full_state", use explicit "template_save"
* use RAII/Unwind to skip saving automation-state

30b087ab3d globally change all use of "frame" to refer to audio into "sample". 
Generated by tools/f2s. Some hand-editing will be required in a few places to fix up comments related to timecode
and video in order to keep the legible

f8abdd9cdf Remove LocaleGuard from ARDOUR::MonitorProcessor state method 
All float <=> string conversions are done using PBD::to_string/string_to via
XMLNode and LocaleGuard is not necessary.

e9a8ccc7e2 major internal plugin & processor API change: 
Pass current (latency compensated) cycle times to plugin.
This fixes time-reporting to plugins and also fixes automation
and when bouncing (the session->transport* is not valid) etc.

5531c83496 change API of Controllable::Changed signal to include (from_self, GroupControlDisposition) 
This allows the signal to convey more information, which may be required by some handlers of a control's Changed signal

9e3299f97d change Controllable::set_value() API to include grouped control consideration. 
This also removes Route::group_gain_control() and associated machinery.
Not yet tested with Mackie or other surfaces. More work to done to
start using the group capabilities, and also potentially to add
or derive more controls as RouteAutomationControls

46f97e8d92 Initial Gain Coefficient tweaks 
1)  Disambiguate 1.0 to GAIN_COEFF_UNITY, and 0.0 to GAIN_COEFF_ZERO
2)  Add GAIN_COEFF_SMALL which replaces SMALL_SIGNAL (-140dB)
3)  GAIN_COEFF_SMALL can used to avoid interpolating towards -inf on a db scale
4)  GAIN_COEFF_SMALL is used to detect very small (denormal?) gains and memset to zero

83519faffb low-pass filter gain-fader. 
fixes various fader zipper noise issues.
It voids sample accuate fader automation (the fader-gain
is low-pass filtered at 10Hz). Yet all musical purposes this
makes a lot more sense than sample accuracy anyway.

bfd50cdeb0 rework MIDI [processor|plugin] chain 
* forward midi-data around plugins that have no MIDI-out
* allow to insert plugins with no MIDI-input at a point with one MIDI-channel

This works because excess ports (both plugin and route) remain
unconnected and use scratch-buffers.

Tested with LV2, LXVST and LADSPA.
(AU plugins with variable in/out retain the old behavior, no bypass)

500aaa0dee an entire freakin' day working on 1 working function, VolumeController::adjust() ... probably not done, but better ... gain control that occurs via this function occurs in discrete, fixed dB steps

13232d03f3 Modify route _processor list set up so that the logic for placing `invisible' processors (e.g. internal returns etc.) is in one place. Add option to get pre-fade listen from before or after pre-fade processors (#3781).

7b6b75f38f forward port automation handling changes from 2.x, upto and including about rev 6981 (will need full testing in the 3.X context). as on 2.x, this removes real-time visual updates to automation curves during write/touch automation recording

ece5093234 change Control::{set,get}_float to Control::{set,get}_double and make almost all Control-related stuff use double rather than random mixtures of float, double and some integer value ; make hans' program change stuff work quite a bit better (not finished yet) including keyboard edting of pgm change values

325671e20a how about that ... a monitor/main section .. GUI is still unfinished .. several small fixes to processor loading/listen mgmt and a few debug output lines rmeoved. knob images are provisional, and can be found in icons/knob.png and related files
