switch from boost::{shared,weak}_ptr to std::{shared,weak}_ptr

This is mostly a simple lexical search+replace but the absence of operator< for
std::weak_ptr<T> leads to some complications, particularly with Evoral::Sequence
and ExportPortChannel.
